Life Care Center at Wells Crossing has been a true blessing for our family. From the very beginning, Allison, Amanda, and Charlene worked quickly and with such dedication to make sure our loved one was admitted smoothly.

Their professionalism, kindness, and genuine compassion gave us peace of mind during a stressful time. It’s clear they love what they do, and it shows in the way they treat families. We couldn’t be more grateful for their help and care.

Excellent staff! They are helpful, extremely knowledgeable, most importantly caring and compassionate. The facility is nice, clean and the day rooms have a lovely atmosphere with a view of a wonderful courtyard and small lake.

I have been in 5 rehab centers in the last two years and this is by far the best. The nurses and cans are very attentive and pleasant. I have not met one employee here that doesn’t seem to be happy with their employment. At the other rehab they complain in front of patients and residents.
I am much better in the two weeks I have been here, PT and OT are awesome. This is a great place for getting well. If I ever have to be a long term resident, this is where I would like to be. Thanks for great care and respect. Debbie Millhorn April 2025.

Although the facility was very dated at first glance, the staff was tremendous. Our son was there for short term rehab and the PT team accomplished a lot in a very short amount of time.Charlene Crawford helped us to check in and was always available to answer our questions and be supportive.

Great facility. Director Tim Hall is hands on with all his management and staff. When I leave everyday I know my wife is taken care of and she is very happy with Life Care Center at Wells Crossing in Orange Park.

Having seen pts across nursing homes in various parts of the country, I say Life Care of Wells Crossing is the ideal place to put your loved ones if needed as well as for Short term rehab. I appreciate the care at this place where it is realized by both therapists and nursing that this is pts new home not just a work place for them. Keep the good work and don’t forget that it can always be better.

I give Life Care Center 10 stars,they do a great job taking care of my mother. This our 2nd time in this facility and I highly recommend using them to take care of your love ones. The Best place and always clean and the staff is very friendly as well as helping and Pleasant to family members.

By far the best facility I have ever been in, they treat you like family. The CNAs are awesome especially Lisa she is very attentive and caring. And I could not say enough about Jewel the social worker, she is the best!
