My Mother has been at Provident for almost 7 months. There was a change in director's after my Mom had been there about a month. Jenny, the new director, is great. Nurse Brandi Burnett is awesome. I don't know all of the staff's names but the ones I've been in contact with or spoke with on the phone have all been very nice and helpful; Brandi, Genette, Nicole, Jennifer, Heidi, Carol. My Mom's health has been up and down but staff has taken good care of her. They always call, email or text when any change or problem arises. My Mom is under Hospice care and they are wonderful. I get weekly updates on my Mom's health.
The facility is always clean and if there is a mess someone is always quick to clean up. The decor is nice, very appropriate.
A hair stylist came a few weeks ago and residents were able to get their hair cut & styled, etc. My Mom's hair looked great.
I visited my Mom on May 5, Cinco de Mayo, and they served Nachos with rice and beans with little non alcoholic Margaritas. The dining room was decorated for the occasion. My Mom ate 100% of her meal.
My brother & I have no complaints. We're thankful that a place like this exists for people with Alzeimers.
I would just like to add that when I visited my Mom on May 13th that landscaping was taking place. When I got there a man was putting out mulch & rose bushes had been planted. Brandi Burnett & her kids were also helping plant flowers, trimming bushes, etc. It is beautiful!!!! The day was so nice that I took my Mom outside on the front porch. Other residents & staff came out also. Destiny & Holly are great with my Mom & the other residents, very kind, patient & respectful.

My mother was a resident at Provident Memory Care for approximately 18 months. Initially, I was apprehensive about moving her there because the overall appearance of the facility was a bit shabby. Pictures on the wall were hung haphazardly, the landscaping was in need of TLC and the place just did not look clean. Unfortunately, we were told that it was the only memory care facility in Livingston that could house my mother because (due to advanced dementia) she was an escape risk.

Due to COVID restrictions, we were forced to do window visits. To stand outside her window, we had to step in standing water and wade through weeds in the back yard of the facility. Once during a visit, I noticed that the rain gutters were deteriorating, allowing water close to the building and there was ALWAYS standing water outside my mother's window. Also, the staff did not always know where the key to the gate was located. During one visit, the staff member allowed me to walk through the dining hall to get to my mother's window because she could not find the key to the gate.

My mother lost 15 lbs during her stay there and developed kidney failure because the staff would not help her eat or drink enough. We had to call in Hospice to make sure she was being cared for properly.

One thing we did not know is that in this facility, the "nurses" weren't actually registered and that anyone off the street can be hired to care for the residents there.

Finally, my mother fell twice during her last month there. The first fall fractured her arm and the second one caused a huge knot on her head, fractured two vertebrae and broke her femur just below her hip. She fell during the middle of the night and we were not notified of the fall until late the next afternoon.

When I went to get my mother's things to move her to a different facility, I found that her room had flooded during a thunderstorm the night before. There were towels shoved up against the walls and under the doors. I wonder how often that had occurred that we were not told about? We also were not able to retrieve all of my mother's belongings.

I do know that the new staff members are attempting to make the facility more presentable, but they have a long way to go. The office is definitely looking nicer than it did when we first saw it.

I am sad that we had such a horrible experience. I feel that the staff is earnestly trying to improve the facility, but they are not being given the tools to make it what it should be.

My dad has been at Provident Memory Care for about a month. He really likes it. Brandi and her nursing staff treat him with the dignity and respect that he deserves. They are amazing. Jenny did an outstanding job helping me navigate through the admitting paperwork and has always been there for me, within minutes, whenever a have questions. Thank you so much for the professional and personal care everyone has given to my dad.

My mother has been at Provident for 1 1-2 yrs. She has gotten excellent care the entire time. If there is any problem I get a phone call. Before she was on hospice the nurse practitioner was notified immediately of any health problems. It takes a special kind of person to deal with Dementia and Alzheimer’s patient’s and they are wonderful. Yes the building is older and the furnishings and pictures are old fashioned but so are the patients. They seem very happy and well taken care of every time I’ve visited. Mom got a haircut a few weeks ago after Covid restrictions have lifted some and they could let a hairdresser in. They also do projects, movies and church services. We have been very pleased with the facility.

The nurses are very un-professional. They intentionally agitate the patients, and even remove the batteries from the patients call buttons, so that they if a patient needs help, they will be ignored. Food is taken away from them just because the patient is eating too slow. Head nurse spends the majority of her time on the computer planning her family vacations. Too many incidents where the patients are neglected, and end up with bruises, and too many incidents of patients falling. That's neglect! I have seen where a patient was forced to sit in her own urine and feces while eating dinner. The girls that work there are too immature to be assisting these patients with dementia. I'm afraid that the lady that runs this place"Sandy", is manipulated by the head nurse "Karen, and Karen's daughter "Kierra". I would think twice before I ever let any of these people look after my loved one. In fact, I have a loved one there now but there is nothing I can say or do about her circumstances because it is simply out of my hands. Oh, and the cook that prepares all the meals does not handle the food properly. My honest opinion; this place needs to be shut down.

The horrible treatment my husband received was the worst as it was not any care at all. He died from lack of care and so many falls that caused brain bleed and never was sent to hospital.He was ther for six weeks and lost twenty pounds and also got scabbie on both feet. The place has to have them as he did not have them when he went. They have to be infected as they are very contagious.He was always dirty and smelled of urine and did not wear his clothes. When I went to pick up his things they even gave me some ones else’s belongings. One of which was a crate that had dried urine and a used cathider inside.

Excellent care. My complaint is one patient that does not have Dementia of any type that constantly belittles, puts down and laughs at the other patients. We are considering moving my Dad for this one reason. Today she called him dumb and said he gets dumber every day. Talk about wanting to tell someone off...at one time his IQ was over the charts. This disease is devastating...then for them to be talked to like that by someone who couldn't possibly have any idea.

My mother has been there for almost one year. The patients and facility are kept very clean. My mother likes all of the staff that works there. It is a good feeling knowing that your mom says that she is happy there and that everyone there is nice to her.
